If breed isn't known, please write what the animal most resembles (e.g. "long haired cat" or "lab mix").
Please provide both the pet's name as well as any nicknames that you use.
Please describe the animal's characteristics such as color, size, coat length, tail length, and/or any unique markings (e.g., spots, one white leg, etc.).
If you don't know this number, you can call your veterinarian as they will often have it in their records.
Please tell us how your pet behaves (friendly with strangers, fearful, afraid of men with hats, likes/hates other dogs, etc).
Please advise if your pet has ever bitten anyone or another animal, or if your pet is ever known to act aggressively toward people or animals.
